DESCRIPTION OF WORK
The Care Manager 2 position is responsible to maintain, create and update on-going care plans and in-home assessments, within a variety of care management programs. The goal is to provide supportive services while encouraging independence.
REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
Qualifications:
+ Must meetPA residency requirement
+ Minimum Experience and Training Requirements -Six months of experience as an Aging Care Manager 1, County Caseworker 1, or County Social Casework Intern; OR one year of experience in public or private social work and a Bachelor's Degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences; OR a bachelor's degree with a social welfare major; OR an equivalent combination of experience and training including successful completion of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences.
+ Hiring preference for this vacancy will be given to candidates who live within Armstrong County. If no eligible candidates who live in Armstrong County apply for this position, candidates who reside in other counties may be considered.
